Cutting no-shows by 38% across a multi-location clinic group
Reception staff were spending half their day on phone reminders and still losing a third of bookings to no-shows. WhatsApp automation and an AI voice agent absorbed both problems.

The challenge
Across six locations the group was losing close to a third of booked appointments to no-shows, and reception staff estimated they spent more than half their shift on the phone — either chasing reminders or missing incoming calls while doing so.
An audit of the phone system found 47 missed calls a day across the group, mostly at lunchtime and after 6pm. Every one of those was a potential booking that went to a competitor.
What we did
WhatsApp Business API was provisioned for the group, with templates approved for booking confirmations, 24-hour reminders and rescheduling links. Opt-in was captured at the point of booking.
An AI voice agent was configured to answer calls that went unanswered after four rings, at lunchtime and outside staffed hours. It handled availability questions, booked appointments directly into the clinic management system, and transferred anything clinical to a human.
Both were wired into the existing practice-management software so nothing required double entry.
The strategy
The reminder sequence was deliberately simple: a confirmation at booking, a reminder 24 hours before with a one-tap reschedule link, and a follow-up two hours after a missed appointment offering to rebook.
Making rescheduling one tap rather than a phone call turned out to matter more than the reminder itself — a meaningful share of former no-shows became reschedules instead of lost revenue.
The voice agent was piloted at two locations for three weeks before rolling out to all six, with transcripts reviewed twice a week to tighten its responses.
The results
No-show rate fell from 31% to 19% within three months. Missed calls dropped from 47 a day to 3.
Reception time spent on administrative calling fell from around 38 hours a week across the group to 11, which the group redirected to in-clinic patient handling rather than reducing headcount.
The reschedule link accounted for the largest share of the improvement — most of the recovered appointments were people who had intended to cancel and previously simply did not show.
